1. What age group do you support at Rise Up Care?

We support young adults aged 18–35 with learning disabilities, providing tailored care plans and experiences that promote independence, social skills, and personal growth.

Rise Up Care was formed and created to ensure that young people at similar stages in their lives were together to help to form lifelong friendships and build their skills up in an age appropriate way. 

2. What services do you offer?

We offer a wide range of services, including:

Day & Overnight Respite Services

Personalized Care Plans

Life Skills Development (e.g., travel training, independent living, e-safety)

Work Experience Placements (with dedicated staff support)

Annual Residential Trips

Social and Recreational Activities

3. Are your staff members qualified and experienced?

All Staff (and we mean all) are trained and certified in the following areas:

  • DBS Checked
  • Makaton Level 4 Certified
  • Safeguarding Level 3 Certified
  • Emergency Administration of Buccal Midazolam
  • Gastrostomy Administration & Maintenance
  • Emergency First Aid & Basic Life Support
  • Crisis Prevention Institute – Safety Interventions
  • Multitude of other learning disability awareness course
  • Experienced and most importantly, KIND!

4. How are services funded?

Our services are commissioned by Torbay Council and the NHS. We work closely with families, healthcare professionals, and local authorities to ensure tailored support that meets each individual’s needs. We will also accept privately funded care arrangements. 

5. How do I arrange a visit or get started?

6. Can I book overnight stays?

Not one off stays, overnight stays are usually commissioned and approved by the social care department (e.g. Transitions Team, Adult social care) as part of a care package. (For example, one day a week). As with the day services we can also accept privately funded care arrangements 

7. What activities do you offer during day services?

We provide a variety of social, recreational and skill-building sessions designed to encourage independence, confidence and happiness!

General activities and skill areas covered are:

  • Independent Living Skills (Cooking, cleaning, Managing a Home)
  • Travel Training
  • Community Access
  • Developing Hobbies and Interests
  • Creative Sessions
  • Social & Emotional Health and Wellbeing 
  • Days out (Further afield)

8. How do you develop personalized care plans?

We work closely with each individual, their families and relevant professionals to create tailored care plans that address immediate needs and long term goals. We take our time to get this right, there’s a bit of paperwork, lots of cups of teas and informal meetings and time spent getting to know our clients where we decide together the goals and areas they want us to work on to help them grow in confidence.

This may be something practical such as life skills to prepare for supported living but it could also be more experienced based such as developing friendships and meeting more people and finding out what hobbies and interests in the local area our clients enjoy.

In general, our care plans will cover a multitude of areas and there won’t be one goal but many goals of focus so that we are accountable when we are providing care to you and there is a clear expectation set out for the care you want delivered by us.

Care Plans are reviewed monthly and we have daily check ins with clients to make sure everything is ok, change anything we need to and most importantly that they are happy!
